Effective antiretroviral therapy turned HIV from a fatal illness into a manageable chronic condition, and in doing so it changed what "living well with HIV" means. The old concerns - wasting, lost muscle, a collapsing immune system - have largely given way to the problems of long-term treatment: fat redistribution, disturbed lipids and insulin, bone loss, and the cardiovascular and mood burdens of a chronic disease. Exercise addresses almost all of these, and the pooled trial evidence is unusually clear about what it improves and what it leaves alone.
